This form is for new patient to register at Algester Medical Centre, please note 1 person 1 form must have complete this form before the booked consultation.
If you would like Algester Medical Centre to release your medical record to yourself or your new GP, you will need to go thru this form to carefully answer all the questions before submission along with the contact details of the receivers.
This form only for our existing patients who have recently changed their contact details to complete. (includes: Phone number, Medicare card, email address, Emergency and next of kin changes)
If you would like to register your MyMedicare under one of our GP, please make sure you carefully answer each question. If you have already registered with GP at other clinic, our GPs cannot provide any assessment or to check any eligibility which including Health assessment, Care plans, etc.
