We strive to provide both convenience and excellence in our service. As a result, we have simplified access to pathology and x-ray facilities. Monday through Friday, from 8:00 a.m. to 4:00 p.m., we have QML pathology on-site.

If you would like Acacia Ridge Family Practice to release your medical record to yourself or your new GP, you will need to go thru this form to carefully answer all the questions before submission along with the contact details of the receivers.

If you would like to register your MyMedicare under one of our GP, please make sure you carefully answer each question. If you have already registered with GP at other clinic, our GPs cannot provide any assessment or to check any eligibility which including Health assessment, Care plans, etc.
