To make your visit as convenient as possible, we offer simplified access to pathology and imaging services. QML Pathology is available on-site Monday to Friday from 8:00 a.m. to 4:00 p.m., ensuring you can complete essential tests quickly and easily during your visit.

This form is for new patient to register at Acacia Ridge Family Practice, please note 1 person 1 form must have complete this form before the booked consultation.
If you would like Acacia Ridge Family Practice to release your medical record to yourself or your new GP, you will need to go thru this form to carefully answer all the questions before submission along with the contact details of the receivers.
